Personal Completion of Medical Information

All medical questionnaires, review forms and declarations must be completed personally by the patient.

This ensures prescribing decisions are based on accurate, reliable and first-hand information.

If assistance is required due to accessibility needs, this should be discussed with the clinical team in advance.

Providing inaccurate, incomplete or third-party medical information may result in treatment being paused or discontinued in order to protect patient safety.
